Print sizing rule of thumb: the QR scans reliably from roughly 10× its printed width. A 1.5cm QR on a business card scans from ~15cm. A 30cm QR on a banner scans from ~3m. Use SVG for any print job above 3cm so quality stays sharp at scale.
Color contrast: minimum 60% luminance difference between foreground and background. Inverted (light QR on dark background) works but reduces scan reliability on cheap scanners.
